Pursuant to the passage of SB 520, found here, any Income Deduction Order filed in the State of Georgia will now be known as an Income Withholding Order effective July 1st, 2024. The functionality of the form will remain the same. Likewise, the form previously called the Income Withholding Order (Incoming Withholding for Support, OMB 0970-0154) will now be known as an Income Withholding Notice

Welcome to the Georgia Income Withholding Order Website for Private Cases

This webpage serves the needs of parents, attorneys, and employers with information related ONLY to private child support cases (also known as Non-IV-D cases) that include Income Withholding Orders (IWO) for the withholding of child support and sometimes Alimony. This information is not used by the Division of Child Support Services (DCSS) on cases that agency manages.

Before income can be withheld for child support, a separate IWO must be signed by a judge and filed with the clerk of court. (See Uniform Superior Court Rule 24.11 on this website). The IWO form (see step 1 below) is required by Uniform Superior Court (USCR) Rule 24.12. The IWO form should not be altered in any way other than providing the requested information.

Income Withholding Orders serve as a withholding process when a parent is ordered by a court to pay child support that will be withheld from his or her pay. Information provided on this website will assist you with correctly implementing a private Income Withholding Order under Georgia and federal law in new orders, when orders are modified or enforced, or when the non-custodial parent changes jobs.

The DCSS database is used by the FSR for posing payments and for the distribution of payments in private (Non-IV-D) cases. Neither the DCSS nor the FSR can assist you with these forms or help with managing your Income Withholding Order. Private Income Withholding Order cases are managed only by a parent and/or an attorney representing a parent. 

Important Exceptions
If the order in the private case is to collect alimony only or pension payments, money deducted by the employer, or the pension entity must be paid directly to the appropriate parent and cannot be paid to the Family Support Registry (FSR). Do not register an Alimony only or Pension only case with the FSR.
Forms and Processes in Private Cases

The steps below include links to forms available for your use.

Step 1 – Prepare and send these documents to the Family Support Registry 

a. FSR Registration Form

b. A copy of your Income Withholding Order (IWO) (fillable PDF)


While not required, it is helpful if you also send a copy of your Child Support Order.

Important!
Fax documents to 770-836-2701, OR mail documents to the Family Support Registry (FSR), P.O. Box 1800 Carrollton, GA 30112-1800. Under Georgia law, all payments made by Income Withholding Order are paid to the Division of Child Support Services Family Support Registry (FSR). A record is made of each payment and it is forwarded to the custodial parent.

Important!
Do not file the IWO with the Superior Court Clerk’s office! This form contains a field for the obligor’s Social Security Number (SSN). Filing a document with a SSN violates Georgia law.
