Request Form for Protected Persons
As authorized by OCGA § 15-5-112, any current or former judge or justice of this state, judge of any county or municipality of this state or the United States, and their spouse may request that a state or local government entity restrict any personally identifiable information (PII) from public disclosure. PII is any personal phone number, home address, or property or tax records of an identified person. This process was established by House Bill 199 in the 2025 Legislative Session and was effective July 1, 2025
There may be consequences to restricting information from a public record, which is a risk undertaken by the requestor, including possible delays insuring title for real estate transactions. Once the restriction is requested and completed, additional action by the requestor may be required to make the restricted information publicly available and could result in a delay in the release of such information. If you wish to release your information to specific individuals or entities, please complete and submit to the appropriate state or local government entity a Request to Release Restricted Information form.
